Endo belly: what it is and how to ease it
If you live with endometriosis, you'll know that 'bloating' doesn't quite cover it. Endo belly is a severe, often painful distension of the abdomen that can make even your loose clothes feel tight.
The short answer
'Endo belly' is a term used to describe the extreme abdominal bloating that often accompanies endometriosis. Unlike normal digestive bloating, it's caused by internal inflammation and the way endometriosis affects the pelvic organs. It can be physically uncomfortable, emotionally draining, and can last anywhere from a few hours to several days.
Why it happens
While the exact cause can vary between individuals, endo belly is usually a result of the body's inflammatory response. Endometriosis tissue — which acts like the lining of the womb but grows outside of it — bleeds every month, just like a period. This blood has no way to escape, causing irritation and inflammation.
This inflammation can lead to:
- Digestive involvement: Endometriosis lesions on the bowel can cause direct irritation.
- Pelvic floor tension: Chronic pain often leads to 'guarding', where pelvic muscles stay permanently contracted.
- Small Intestinal Bacterial Overgrowth (SIBO): Research suggests women with endometriosis may be more prone to gut flora imbalances.
- Adhesions: Scar tissue can cause organs to stick together, hindering normal movement and digestion.
What may help ease the pressure
Finding relief for endo belly often requires a multi-pronged approach. What works for one person might not work for another, but here are common strategies:
Dietary tweaks: Some find that a low-FODMAP diet or reducing inflammatory triggers like dairy and gluten can help reduce the severity of flares. It's not about restriction, but about finding what keeps your gut calm.
Gentle heat: A warm hot water bottle or a magnesium-rich bath can help relax the muscles of the abdominal wall and pelvis.
Anti-inflammatory support: Because inflammation is at the root of the bloating, many women look for ways to support their body's response. Localised CBD delivery, such as through a vaginal pessary, can help calm the pelvic area and relax tense muscles without the need for digestive processing.
When to see a doctor
Bloating is common, but it's important to know when it needs medical investigation. You should speak to your GP if:
- The bloating is persistent and doesn't go away.
- You experience severe, sharp, or sudden abdominal pain.
- You have a fever or feel generally unwell alongside the bloating.
- You notice significant changes in your bowel or bladder habits.
Remember, your pain is real and you deserve to be heard. If you're struggling to manage endometriosis symptoms, keep a diary of when the bloating occurs to help your doctor understand the pattern.
